The Interdisciplinary Team (or IDT) is one of the unique features of Maui PACE. The IDT assesses need, provides care, coordinates contracted services and monitors your situation. You and your family and caregivers work together with the IDT to create an individualized plan of care. This plan is a holistic approach to managing care.
The Maui PACE IDT consists of a physician, a registered nurse, a social worker, a physical therapist, an occupational therapist, an activities professional, an in-home coordinator, a transportation coordinator, and a dietitian. Having so many different specialties working together helps to ensure that all your needs are met in a coordinated fashion.
Transportation is another important service Maui PACE offers. We provide wheelchair-accessible transportation to the PACE Center and to all medical appointments coordinated through the IDT.
Often, doctors are trying to fit lots of patients into their schedules, leaving them rushed for time with each patient. As a result, patients get less and less time with the physician. The Maui PACE physician is there for you, and spends as much time with you as you need. As a member of our Team, the physician really gets to know you personally.
